Birth Certificate
Birth of a child ought to be submitted during 14 days from birth act.
Birth act is being made up according to the written notification of a newborn child from a doctor, midwife or health care center.

Birth certificate ought to enroll:
1) surname, name (names) and sex of a child
2) place and date of the child birth
3) surnames, family names, name (names), place and date of birth, adress of each parent from the time when child was born
4) surname, names and birthplace of proposer
5) register from health care center if birth certificate is being written based on its admittance

Basic of writing child`s birth certificate beside certificate written by doctor or health care center is its parents marriage certificate if they remain in marriage. Forementioned act certify origin of a child, defines its surname or determines personnal data of parents.
   

Child can be given by no more than two names. They cannot be ridiculing, inappropriate, diminutive or ones by which one cannot describe child`s sex. If during dispensing of birth certificate, parents did not choose name , head of civil registry office enrolls to the birth certificate one of the name that is commonly used in country.
